Requirements:

  1. Passport should contain at least two blank pages.


  2. Zip-code of the address in The Netherlands must be mentioned in the application form.


  3. Attention! Application forms for minors should be signed by both parents. Also it is necessary to provide the copies of data pages of parents’ passports.
