Residence with spouse/ registered partner

• a valid passport;

• a photocopy of the valid passport of your spouse or registered partner (except blank pages);

• (if your marriage was solemnised or your partnership registered outside the Netherlands) your original marriage certificate or certificate of registered partnership. The certificate must be legalised or provided with an apostille;

• the certificate showing that you have passed the civic integration examination abroad (unless you are not obliged to take the examination.

Residence of child under 18 years with parents

• (if your are aged 16 years or older) a certificate showing that you have passed the civic integration examination abroad (unless you are not obliged to take the examination.

• a valid passport (if you have a passport of your own);

 • (in the case of parents who are Dutch nationals) photocopies of every page of the identity documents of both your parents; (in the case of parents who are aliens) photocopies of both sides of your parents’ residence permits or of every page of their passports containing a residence permit stamp or note;

 • If you are a child of a single father or mother or if you are a child from a previous marriage or relationship of one of your parents, you must submit:

 o official documents, such as a foreign judicial decision, stating that your parent(s) has/have parental responsibility for you. Such documents must be legalised or provided with an apostille;

o a declaration of consent concerning your departure from the parent remaining behind;

o a copy of the passport or identity document of the parent remaining behind;

• (if you are aged 15 years or older) a declaration signed by yourself that you are not married or responsible for caring for any children of your own;

• (if your are aged 16 years or older) a certificate showing that you have passed the civic integration examination abroad (unless you are not obliged to take the examination.

Residence of child older than 18 years with parents
(family reunification)

• a valid passport;

 • a declaration establishing your unmarried status. This declaration must have been issued within the past six months and must be legalised or provided with an apostille.

• a document showing evidence of a family-law relationship between you and the persons with whom you want to reside. This document must be legalised or provided with an apostille;

• the certificate showing that you have passed the civic integration examination abroad (unless you are not obliged to take the examination.

• a valid passport;

Residence of a single parent (65 years) with child

Visiting relatives for more than 3 months

• a valid passport;

a document showing evidence of a family-law relationship between you and the relative you want to visit. This document must be legalised or provided with an apostille.

 • a document showing evidence of a family-law relationship between you and the relative you want to visit. This document must be legalised or provided with an apostille;

 • a photocopy of your return ticket or other documents showing that you have sufficient funds for your return journey.

Residence as foreign foster child

a valid passport;

 • photocopies of both your foster parents' passports (except blank pages);

• a document showing evidence of a family-law relationship between yourself and your foster parents. In so far as is applicable, this document must be legalised or provided with an apostille;

• a medical certificate describing your state of health;

• (if you are aged 15 years or older) a declaration signed by yourself that you are not married or responsible for caring for any children of your own;

 • (if your are aged 16 years or older) a certificate showing that you have passed the civic integration examination abroad (unless you are not obliged to take the examination.

• a valid passport;

 • photocopies of both your foster parents' passports (except blank pages);

• a document showing evidence of a family-law relationship between yourself and your foster parents. In so far as is applicable, this document must be legalised or provided with an apostille;

 • a medical certificate describing your state of health;

 • (if you have living parents or legal representatives) documents showing that your parents or legal representatives have agreed to you coming to reside with your foster parents in the Netherlands; (if you have no living parents or legal representatives) documents from the competent authorities in your country of origin showing that they have agreed to you coming to reside with your foster parents in the Netherlands;

• a written explanation of the special circumstances in which you or your family find yourselves in the country of origin, showing that you cannot be cared for appropriately by your family in the country of origin;

• (if you are aged 15 years or older) a declaration signed by yourself that you are not married or responsible for caring for any children of your own;

 • (if you are aged 16 years or older) the certificate showing that you have passed the civic integration examination abroad (unless you are not obliged to take the examination.

Residence as adopted child

a valid passport;

• the original official decision given by the competent authorities in the country of origin, together with a translation of this document (if applicable). This document must show that the authorities in your country of origin have agreed to your being taken into your adoptive family. In so far as is applicable, this document must be legalised or provided with an apostille;

• documents showing that your natural parents have relinquished parental responsibility for you (their agreement may be evident from the document referred to above).

• (if you are aged 16 years or older) the certificate showing that you have passed the civic integration examination abroad (unless you are not obliged to take the examination.

a valid passport;

• the original official decision given by the competent authorities in the country of origin, together with a translation of this document (if applicable). This document must show that the authorities in your country of origin have agreed to your being taken into your adoptive family. In so far as is applicable, this document must be legalised or provided with an apostille;

 • documents showing that your natural parents have relinquished parental responsibility for you (their agreement may be evident from the document referred to above).

 • (if you are aged 16 years or older) the certificate showing that you have passed the civic integration examination abroad (unless you are not obliged to take the examination.

Au pair a valid passport. • your passport.
Student

a valid passport.

• an original declaration by the education ministry in your country of origin that the same study programme in not available there (applicable only to secondary and vocational education). Unless the advisory procedure has established that the funds to finance your study programme have been deposited into a Dutch bank account, or unless your study programme is being wholly financed by a party resident in the Netherlands, you must also submit the following documents:

• documents relating to your foreign bank account showing the balance that will be available to you during the academic year;

• an original declaration by your bank showing that an amount will be transferred for your benefit every month to a bank account in the Netherlands, along with a statement of the amount to be transferred; • an original declaration that you will receive a student grant (if applicable).

a valid passport.

• If you want to study in secondary or vocational education:

 o an original declaration by the education ministry in your country of origin that the same study programme is not available there;

 o a written explanation of why the Netherlands is the most suitable country in which to follow the study programme concerned and how the study programme is likely to benefit the labour market in the alien’s country of origin;

• an original document to show that you will be admitted to the educational institution where you wish to study; or if you have a referee in the Netherlands, a copy of this document;

• if you want to follow a preparatory course for a higher education study programme, an original declaration by the educational institution offering the programme for which you want to prepare. This declaration must show that you will be admitted to the programme after you have successfully concluded the preparatory course; or if you have a referee in the Netherlands, a copy of this declaration.

• Unless the funds to finance your study programme have been deposited into a Dutch bank account, or unless your study programme is being wholly financed by a party resident in the Netherlands, you must also submit:

o documents relating to your foreign bank account showing the balance that will be available to you during the academic year;

o an original declaration by your bank showing that an amount will be transferred for your benefit every month to a bank account in the Netherlands, along with a statement of the amount to be transferred;

o an original declaration that you will receive a student grant (if applicable).

Knowledge migrant • a valid passport.

 • a copy of the declaration submitted by your prospective employer to the Immigration and Naturalisation Department (IND) under the knowledge migrant scheme.

Paid employment • a valid passport.

Work placement • a valid passport.

 • a photocopy of the letter of invitation from the organisation or enterprise where you want to undertake your work placement.

Self-employment • a valid passport.

If you have no referee in the Netherlands, you must submit the following documents:

• a recent original extract from the Chamber of Commerce showing the registration of your enterprise (unless such registration is inapplicable, as in the case of certain professions);

• (if necessary) a photocopy of a document, issued by a competent Dutch authority, showing that you are qualified to practise the profession concerned or operate the enterprise concerned;

• a photocopy of a business plan, containing:

 o information about yourself (your personal details, family and income, financial obligations, education and training, and occupational experience);

o information about your enterprise (the field in which you intend to operate, the date on which you intend to start operating, and the address at which you intend to establish your enterprise);

o legal information (your enterprise's legal form, trading name, necessary licences, liabilities, insurance, and terms of delivery);

 o commercial information (the type of enterprise concerned and its products, its innovativeness, the market, the target group, and the competition, supported by contracts and references);

o management information (the enterprise’s organisational structure and size of its workforce);

o financial information (an investment budget, a financing plan, a repayment plan, an operating budget, and a liquidity forecast);

• (if you were also self-employed abroad) a photocopy of the enterprise’s profit and loss account for the past 12 months;

• (if the enterprise is a public limited company, a private limited company, or a general or limited partnership) a copy of the deed of incorporation stating the partners’ powers, responsibilities, and share of profits;

 • (if you, as the enterprise’s managing director and main shareholder, have an interest of more than 25%) documents showing your interest in the enterprise;

• evidence that the enterprise is new (i.e., that the enterprise is entering the market for the first time, that its legal form and management have changed, or that it is radically altering its business activity);

• evidence that the activities you intend to perform cannot be performed by a person resident in the Netherlands or an alien with a residence permit;

• (if you intend to work as a freelancer) documents concerning assignments showing that you have performed freelance activities.

Residence for medical treatment • a valid passport.

If you have a referee in the Netherlands:

• a valid passport.

If you do not have a referee in the Netherlands:

• the name and address of the doctor who is to treat you or the medical institution where you are to undergo treatment;

• an original declaration of consent, fully completed and signed, for the Ministry of Justice Medical Adviser (see annexe). This declaration must have been issued within the past month;

• documents showing that you have adequate insurance to cover your medical expenses in the Netherlands or documents showing that arrangements have been made to pay the expenses of your accommodation in the Netherland and your return journey;

• an original medical certificate completed by the doctor treating you, issued within the past month, stating the nature of your illness, how long your treatment will last, and why you need to be treated in the Netherlands. This certificate may not be handwritten;

• if you consider that special facts or circumstances should be considered with regard to your application, please also enclose a declaration stating them. You should support this declaration with as much documentary evidence as possible. If this evidence consists of official documents concerning marital status, they must be legalised or provided with an apostille.

Readmission to Holland N/A

• a photocopy of your passport (except blank pages);

If you have applied to The Hague district court for confirmation of your Dutch nationality:

• a photocopy of your entire application to the court for confirmation of your Dutch nationality (pursuant to section 17 of the Netherlands Nationality Act).

If your residence outside the Netherlands was due to imprisonment, military service, or postings abroad:

• a photocopy of both sides of your old residence permit;

 • documents concerning your imprisonment abroad and its duration, the performance and termination of your military service abroad, or your postings abroad.

If, between the ages of four and nineteen, you resided legally in the Netherlands for at least ten years; or if, before the age of nineteen, you resided legally in the Netherlands for at least five years without interruption:

• information and documents concerning the duration and nature of your previous residence in the Netherlands;

• information and documents showing why the Netherlands is the most appropriate country for you to reside in.

If you are eligible to return to the Netherlands under section 8 of the Repatriation Act:

• a photocopy of the declaration by the Social Insurance Bank (SVB) confirming your entitlement to a basic payment or repatriation allowance under the Repatriation Act and mentioning the date on which you left the Netherlands;

 • a photocopy of the accompanying annexe, showing the date of commencement of your previous residence permit for long-term purposes (long-term purposes include residence with a partner, relatives, or immediate family) and the uninterrupted duration of your previous period of residence in the Netherlands.

If you were originally a Dutch national and lost your Dutch nationality under section 15 of the Netherlands Nationality Act because you voluntarily acquired another nationality or because your Dutch nationality was withdrawn:

• information and documents concerning the duration and nature of your previous residence in the Netherlands;

 • an original extract (not a photocopy) from the municipal personal records database (GBA) mentioning the date on which you relinquished your Dutch nationality;

• letters from the Minister of Justice informing you of your obligation to relinquish your Dutch nationality.

If you lost your Dutch nationality:

• a photocopy of every page of your old Dutch passport.

If you acquired another nationality voluntarily:

• a photocopy of the document confirming your naturalisation.

If your Dutch nationality was withdrawn:

 • a photocopy of the decision concerning the withdrawal of your Dutch nationality.

Residence on special grounds

• a valid passport.

 If you do not have a referee in the Netherlands, you must also submit:

• an account written in Dutch of why you should be granted an authorisation for temporary stay. In other words, what are your reasons for wanting to reside in the Netherlands? And why do none of the reasons for residence summed up in article 3.4, paragraph 3, of the Aliens Decree 2000 not apply to you? (*). You should support your account with as much objectively verifiable documentary evidence as possible. If your documentary evidence consists of foreign official documents concerning marital status, these documents must be legalised or provided with an apostille.
